Program Objectives

Target Audience
  • Physicians: Physicians can benefit from the Consumer Health Index (CHI) because poor lifestyle habits put them at high risk for cardiovascular disease.



  • Consumers: Consumers can be targeted through healthcare professionals (physician offices, pharmacies, hospitals, etc.) as well as through membership organizations, employers, health plans, and media channels.


Program Description
  • Online health risk assessment tool for patients: The CHI tool is the first of its kind to use Body Mass Index (BMI), Family History and Lifestyle to quantify and stratify cardiovascular health risks. The assessment is short and generates a real-time personal health report.



  • The CHI tool is designed to efficiently collect demographics, family history, lifestyle, diagnosed disease, drug utilization and permission for longitudinal health management, communication and messaging.



  • The CHI tool only requires height and weight inputs to generate a personalized report. All other inputs can be customized (added or deleted).



  • Free Pass Enhanced Solution: Additional lifestyle and disease management programs can be added to a voucher for the CHI tool (see right) to provide additional value to the patient. This includes online nutrition and fitness programming, online weight loss programming, as well as telephonic/computer aided health coaching.



  • The CHI has been validated by Dr. Gary Foster, University of Pennsylvania School of Medicine & President of NASSO – The Obesity Society.

  • Physicians: CHI Vouchers are printed and distributed to patients and/or consumers through the following channels and/or advertisements to create awareness of a website through which the CHI tool can be accessed:

    5. Retail Pharmacies (NCPA)
    6. Health Screenings (CHI Voucher included with blood pressure check)

Cost
